PS. Since there seems to be some confusion about the Clinic Registration 
form, allow me to restate that you should use the CBC Membership form  
in lieu of a Clinic specific form.

the back of the form, and your check made out to '2009 CBC CLINIC' for 
$50 ...".  Thanks for your cooperation.  I have received 2 registrations 
as of today's US mail.
